26-258 : Be Well Healthcare Medicine Contract Extension

This item extends the contract between the City of Buffalo and Be Well Healthcare Medicine, PLLC (d/b/a Great Lakes Physicians Services) for Employee Health and Wellness Medical Consultant and Injured on Duty (IOD) 207A & 207C Case Management services. The original contract was effective April 1, 2023. This is the third one-year extension, covering the period from April 1, 2026, through March 31, 2027. The services provided include evaluating injured and sick police and fire department employees, acting as a liaison with third-party administrators, coordinating medical examinations, and managing IOD claims. All other terms and conditions of the original April 1, 2023, agreement remain unchanged.
